Collateral EmploymentThe University Board of Trustees and the Ohio Ethics Law require each faculty member to file and submit a request for approval to perform outside activities. Form A-128 (a) should be used by all full-time faculty and librarians; Form A-128 should be used by all unclassified staff and part-time faculty. Faculty completing these forms should explain, in detail, the nature of the outside activity (this no longer includes time spent seeing patients in the department's clinical practice), the total amount of time involved and dates. All full-time faculty and librarians are required to file Form A-128 (b) annually, whether they filed A-128 (a) or not. Form A-128 (b) should summarize all outside activities performed in the preceding academic year. This includes a brief description of the activity, the name of the organization or person for whom the activity was performed, time spent on the activity, and any uses of University resources. In an effort to simplify the process to all those that have no other employment and currently put "none" on the forms, in lieu of A-128 (b), use the Collateral Employment Form.
